Participant Registration Form

The application should be filled out completely and submitted with payment.

  • Sections 1, 2 & 3 - to be completed by the applicant.
  • Section 4, the pastor's statement and signature should be attempted to be completed so that the applicant's pastor is aware of the applicant's attendance at the weekend. However, if a pastor's signature can not be obtained, the applicant CAN still come on the weekend. Therefore, a pastor's signature for the application is recommended, but not required.
  • Section 5 - to be completed by the sponsor
  • Weekend Fee - to be attached with the application when submitted to Pre-Weekend Chair.

Team Registration Form

The team application should be filled out completely and submitted to the Head Cha with full payment. A pastor's signature is required for a team member's application.

Scholarships are reserved for candidates, so if a team member is having difficulty paying for the full team fee they can consider the following options:

  • Obtain a sponsor​
  • Request help from your church
  • Request help from your reunion group
  • Talk to the Rector/Rectora

Our aim is to be transparent in the conduct of the business of Abundant Life Tres Dias.
